Application Checklist
Items you need to provide for application to progress:
- Proof of Legal Name, Date of Birth and Residency Status (e.g. a certified copy of your passport or birth certificate (not drivers licence))
- Certified copies of your results/certificates/registration (if required by programme entry criteria)
- Certified copies of your English Proficiency Test results (if applicable)

If you are applying to enrol in:
- Any Centre for Health and Social Practice programme, you must complete the forms below and supply the supporting documentation as outlined on the forms:
- Master of Nursing or Postgraduate Certificate in Nursing
- Bachelor of Nursing
- Bachelor of Midwifery
- Cervical Smear Takers
- Graduate Diploma in Supervision
- Bachelor of Applied Social Science (Counselling)
- Bachelor of Applied Social Science (Social Work)
- National Certificate in Mental Health (Mental Health Support Work) (Level 4)
- 2 x Referee Questionnaires
- Any Early Childhood Education programme, you must complete the forms below:
- Any Media Arts programme, you must include supporting documentation as outlined in the following Media Arts brochure:
- A School of Sport and Exercise Science programme,you must supply:
- Diploma in Personal Training (Level 5)
- Submit a written personal statement as to why you wish to undertake the programme *
- Provide two written references *
- Have a current Comprehensive First Aid Certificate (or acquire one within three months of commencing the programme), and
- Demonstrate a level of fitness sufficient to fulfil the practical requirements of the programme.
NB * Except for candidates who wish to pathway from the Wintec Certificate in Fitness Industry Training (Level 4)
- Certificate in Fitness Industry Training (Level 4)
- Have a current Comprehensive First Aid Certificate (or acquire one within three months of commencing the programme), and

Frequently Asked Questions
How can I prove my Legal Name, Date of Birth and Residency?
If this is the first time you have enrolled in a programme/module(s) you can prove your details by providing:
- Your birth certificate or passport for New Zealand, Niue, Tokelau or the Cook Islands; or
- A certificate of citizenship for New Zealand, Niue, Tokelau or the Cook Islands.
Students may supply other evidence of their legal name, such as a marriage certificate, although evidence of citizenship will still be required.
A whakapapa statement may be provided by students who identify themselves as Māori if they have a justifiable reason for not providing a birth certificate, for example, if the birth was not registered. In that case, students may provide an original or certified copy of a statement of whakapapa countersigned by a Kaumātua as evidence of their identity and citizenship.
Occasionally, a student is recorded as a child on their parent's passport. In that case, the student may use the parent's passport as evidence of identity and citizenship.
What is a Certified Copy?
A certified copy is a photocopy of the original document, which is signed by a Solicitor, Justice of the Peace or authorised Wintec person e.g. a Student Consultant in the Student Enrolment and Information Centre, as being a true copy of the original.
